St. Francis Memorial Hospital is a 20 bed, fully accredited facility, offering medical, and complex continuing care. Special inpatient services include palliative care, diabetes clinic, telemedicine and Cardiac Rehabilitation. A two bed Hospice is directly down the hall from our inpatient unit. The hospital has an extensive ambulatory care program with over 10,000 emergency visits per year. A six station satellite hemodialysis unit is located at the St. Francis Memorial Hospital in Barry’s Bay.

SFMH offers comprehensive in-patient, out-patient and telehealth clinics in addition to regional programs such as Addiction Treatment, Diabetes Education, Hemodialysis & Nephrology and Ontario Breast Screening. There is full diagnostic support within the hospital including staff radiologists providing radiological, ultrasound, bone density and mammography support. The hospital is located in the village of Barry’s Bay and serves a catchment of approximately 10,000 population dispersed over a large geographic area.

In 2019, SFMH implemented a new state of the art electronic health information system in collaboration with five regional partners including The Ottawa Hospital, Hawkesbury General Hospital, Renfrew Victoria Hospital, Deep River Hospital, Kemptville District Hospital, Winchester District Hospital and the University of Ottawa Heart Institute, as well as the Ottawa Hospital Academic Family Health Team. As part of the health information system, each health partner has real-time access to patient medical information allowing health-care providers to make timely, informed decisions based on the most comprehensive, and connected information.

SFMH has been designated as Best Practice Spotlight Organization (BPSO) by RNAO and through these collaborations and integration efforts SFMH also remains on the leading edge of practice. Yes.  SFMH offers mentorship and cross training opportunitites.  In addition, all SFMH staff can apply for education sponsorship to advance skills and knowledge and reimbursement for required certifications such as ACLS, PALS and others.

Pay Grid: $27.61 - $29.20 /hour

Working under the direction of the Integrated VP Clinical Programs and Ambulatory Care Working as a member of the clinical team and reporting to the Hospice Clinical Director & Chief Nursing Officer, the incumbent provides and supports patient care delivery for both the hospice and hospital (on the same night shift).

Working as a member of the clinical team and under the direction of the Registered Nurse/Registered Practical Nurse (primary nurse), the PSW provides basic Nursing Care; assists the patients with activities of daily living such as personal hygiene, food and fluid intake and elimination needs, and mobility as outlined in the Nursing Care Plans specific to each patient. Within the hospital setting, the PSW assists with restorative nursing functions to maintain or improve function and prevent decline or adverse outcomes.

Duties will include but are not limited to:

  • The PSW provides close supportive care to patients and families, and specialized support of specific populations such as: dementia, palliative, and those with life limiting illness/disease.
  • Participate in change of shift report
  • Keep patient’s white board updated with pertinent information
  • Assisting with ambulating, mobilization, proper positioning and maintaining body alignment and/or posture as required.
  • Assisting with mechanical and physical lifts and/or transfers.
  • Observing and reporting changes in the patient’s condition immediately to the Charge Nurse
  • Assisting with bathing and routine personal hygiene needs, including toileting, oral hygiene, hair care, shaving, dressing, etc.
  • Passes meal trays/nourishments to patients and returns trays to trolley.
  • Feed/assist patients as required.
  • Assisting with restorative care activities to support individual patient goals and other rehabilitative measures as directed by a professional.
  • Practice universal precautions and maintain a clean, safe work environment.
  • Provide emotional and social support services to patients and their families.
  • Assisting with, hot and cold application/compresses as directed by a physician or Nursing Care Plan.
  • Takes and records patient weight and height and relays this information to the RN or RPN.
  • Changing bed linens, towels, and performing laundry related tasks.
  • Calming and restraining restless patients, maintaining safety devices as ordered, as per Least Restraint Policy.
  • Providing one-on-one observation and care of designated patients.
  • Assists Registered Nurse or Registered Practical Nurse with dressing changes.
  • Assists with labelling patient’s personal effects, such as glasses, prosthesis, braces, dentures, hearing aids.
  • Obtaining and labeling non-invasive specimens as directed. [i.e. urine, stool, sputum, swabs]
  • Provides appropriate care to promote comfort and safety of patients as outlined in the patient’s Nursing Care Plan. [i.e. restraints, behavioral charting]
  • Answering call bells, and responding to patient’s requests.
  • Cleaning after soils/spills [i.e. body fluids] and arranging for disposal according to Infection Control protocols.
  • Assist with emergency situations as required
  • Awareness of common allergies and related first aid measures.
  • Providing supportive care to Palliative and Hospice patients.
  • Assisting with colostomies, ileostomies and urostomies within scope; delegated as see-one do- one.
  • Observing and reporting all behavioral changes, changes in on-going conditions, and any other information pertinent to the care of the patient.
  • Assisting with other stable delegated tasks as directed by a professional.
  • Applies necessary ointments or creams to affected areas on patient’s skin, applies prosthesis or braces, etc. when required.
  • Assists with new admissions to the unit, providing orientation to the unit.
  • Assists with the patient’s discharge from the unit.
  • Transporting patients and specimens as required as per policy.
  • Preparing deceased patients, and assisting their transport as required
  • Maintaining supplies and equipment as required.
  • Cleaning of equipment.
  • Other responsibilities as per unit profile, in line with identified scope of practice.

Qualifications:

  • Knowledge acquired through the successful completion of a recognized PSW program or what was formerly known as the HCA program. [i.e. Patient Care Assistant, Health Care Aide, Personal Support Worker]
  • Knowledge of department/hospital policies and procedures.
  • Provide close supportive care to patients and families, and specialized support of specific populations such as: dementia, palliative, and those with life limiting illness/disease.
  • Previous Geriatric and Palliative Care experience preferred
  • Current CPR or BCLS certification, level ‘C’ is required.
  • Good communication and interpersonal skills.
  • Ability to understand, interpret and carry out written and verbal instruction.
  • Ability to set priorities
  • Knowledge of Patient and Family Centred Care
  • Ability to work effectively and provide care within a team environment, according to Nursing Care Plan.
  • Ability to work independently and to perform duties within the physical demands of the work area. [i.e. lifting and mobilization of patients]
  • Basic knowledge of established Infection Control and Occupational Health & Safety practices.
  • Preparing deceased patients and assisting with their transport within the facility as required.
  • Knowledge of body mechanics, and safe lift & transfer practices.
  • Basic knowledge of sterility and aseptic technique.
  • Flexible, Adaptable, and able to multi-task
  • Electronic Health Record Inpatient & Registration Basics Training is a requirement (to be completed before first shift)

Under the supervision of the Dietetic Department Manager, the Cook oversees all activities of food preparation and directs the team of Cook Aides and Dietary Aides to meet the requirements for high quality food production for patients, staff and visitors.

Duties will include but are not limited to:

  • Follows standardized recipes and prepares appropriate quantities of food to meet demands and avoid excesses.
  • Is knowledgeable in the use of therapeutic diets and exchanges.
  • Sets therapeutic menus by consulting and adapting standardized meal patterns to the current menu.
  • Follows portion control guidelines for all diets.
  • Ensures that all food served meets departmental quality standards and temperature controls.
  • Responsible for hospital catering, monitoring and delivery of the Meals on Wheels Program.
  • Monitors and evaluates new recipes.
  • Manages working inventory and projects and corrects supply and demand problems when they occur.
  • Follows sanitation standards for the preparation, storage and serving of food.
  • Participates in HACCP Inspection by the Public Health Inspector.
  • Checks equipment and cooking areas regularly to ensure safety and sanitation requirements are met.
  • Follows recycling procedures.
  • Initiates requests for minor equipment repairs and reports major equipment problems to the Food Services Team Leader.
  • Monitors and balances daily staff food sales and deposits the revenue with the Business Office.
  • Monitors the receipt of all grocery orders.
  • Assigns duties to the Cook Aide and Dietetic Aide to coordinate routine and special tasks among the team.
  • Acts as a resource to the other team members.
  • Responsible for staff replacement in the event of absence and/or illness.
  • Plans for and participates in the orientation of new staff.
  • Contributes to and participates in continuous quality improvement initiatives within the department.
  • Reviews and updates patient nutrition profiles.
  • Visits patients to discuss menu selections, or food likes and dislikes.
  • Addresses staff and visitor inquiries or requests and appropriately forwards problems to the Food Services Team Leader.
  • Performs the departmental purchasing and inventory management function as assigned.
  • Is responsible for the food operation when the Food Services Team Leader is absent.
  • Follows Occupational Health and Safety directives in the application of personal protective equipment.
  • Reports accidents and hazardous situations to the Department Manager.
  • Attends WHMIS and Fire Safety and other in-services that apply to the department.
  • Performs any other related duties as assigned.

Qualifications:

  • Cook Level 1 and 2 certification from a recognized educational institution or equivalent certification.
  • Ability to challenge the Red Seal Certification test. 
  • Must have the ability to plan and co-ordinate work assignments for the team.
  • Demonstrated professionalism and ability to exercise judgment and initiative in preparing food, delegating duties and maintaining efficient standards of operation.
  • Two (2) years of work experience preferably in a Health Care setting preferred
  • Demonstrated teamwork, initiative, and customer service skills and adaptability to a changing environment.
  • Demonstrated commitment to learning.
  • Demonstrated verbal, interpersonal and written communication skills – computer skills an asset.
  • Ability to perform the duties within the physical demands of the work area.
